    Grant Project Description: This project would expand an existing successful dental program for low-income adults in western Maryland through a community health worker model in Allegany, Garrett, and Washington Counties. The project would target individuals who are in recovery from opioids and other addictions and have delayed accessing dental services. A Community Health Worker would work with project participants to overcome the social determinants of health that stand in the way of accessing care. Participants would also be screened for somatic health and social support needs.

    Grant Project Description: This two-year grant is being used to expand the capacity of the Allegany County Health Department’s adult dental program and improve the ability of the Health Department to respond to the dental emergency needs of adults going to the hospital ED. CHRC grant funding is being utilized to support staff salaries and supplies.

    Grant and Amount:  17-020 ($350,000) 
    Grant Project Description: This two-year grant supports 
    implementation of the HOMEBUILDERS® model, an evidence based program which aims to increase family engagement in treatment with a goal of family preservation and cost savings for the social service system.  The Center will work closely with the Allegany County Department of Social Services to provide services for children removed from their parents due to substance use.  The program's goal is family preservation when appropriate by increasing access to behavioral health and wraparound services.  Grant funds will be utilized for clinical staff salaries and training on the HOMEBUILDERS® model.

    Grant and Amount: 16-001 ($50,000) 
    Grant Project Description: This two-year grant is being utilized to expand AHR’s existing Dental Access Program and to serve low-income seniors and disabled adults. The program continues Allegany Health Right’s model of community outreach and engaging private dentists to provide dental services at a discounted rate of 50% - 80%. CHRC grant funding is being utilized to support the salaries of a community health worker and dental case manager and to provide discounted costs for dental services to program participants.

    Grant and Amount:15-002 ($10,000 - Additional $90,000 funding provided by WMHS) 
    Grant Project Description: This two-year grant is being utilized to expand the applicant’s existing dental program by the addition of a community health worker to target Medicaid adults who generate ED dental visits at Western Maryland Health System.  The proposal estimates that $24,000 in treatment funds each year will provide dental care services valued at more than $100,000 for up to 100 Medicaid adults for one year. Grant funds are utilized after Medicaid billing/reimbursement is sought, and are being utilized to support salary and dental treatment costs not covered by Medicaid. Work done through this program was highlighted in an American Journal of Public Health article in December of 2016 entitled 'Impact of a Community Dental Access Program on Emergency Dental Admissions in Rural Maryland'.

    Grant Project Description: This two-year grant supported the provision of dental services to disabled adults in Allegany County. The grantee serves as a referral and coordinating agency for underserved and uninsured adults in Allegany County. Funds were utilized to provide dental care services for underserved communities. Allegany Health Right has leveraged its CHRC grant and received a three year $374,000 grant award from HRSA to continue its outreach efforts for uninsured and underserved adults in the community.

    Grant Project Description: This one-year grant supported a “behavioral health learning collaborative” that provides training and technical assistance to providers in the region. Grant funds were utilized to support the start-up costs of the collaborative, which provided access to training and technical assistance and enabled behavioral health providers to participate in Maryland’s ongoing efforts to promote functional behavioral health integration.

    Grant and Amount: LHIC 13-001 ($185,048) 
    Grant Project Description: This two-year grant supported the use of community health workers to link patients to community resources. Grant funds were utilized to create a community resource guide​, to support cultural competency provider training, and to provide access to subsidized transportation services. Under the Healthy Allegany grant, community health workers helped residents make lifestyle changes, follow their health care provider’s instructions, and connect to community resources. The grantee reported making more than 3,000 client visits and more than 1,400 referrals to medical and social services over the duration of the grant.

    Grant and Amount: 12-002 ($135,598) 
    Grant Project Description: This three-year grant supported a collaboration between Tri-State Community Health Center and the ACHD to bring together resources and health care providers at Tri-State’s existing Women's Health Center. Grant funds were utilized to provide women with OB/GYN and postnatal care services through Tri-State providers and home visiting services through the ACHD staff. The grantee reported serving 256 women through 1,361 visits over the duration of the grant.

    Grant and Amount: LHIC 12-001 ($25,000) 
    Grant Project Description: The Allegany County Health Planning Coalition utilized CHRC grant funds to support the training of more than 80 providers to reduce the incidence of opioid prescription drug use and to support activities to reduce tobacco use, prescription drug and alcohol abuse, and promote regular screenings for chronic diseases.  Grant funding also supported the purchase of nicotine replacement therapy resources to support the health department’s tobacco cessation program, breathalyzers to the County Sheriff’s Office, and Medreturn units to provide permanent medication/drop off sites throughout the community.

    Grant and Amount: 11-003 ($127,000) 
    Grant Project Description: This two-year grant supported a program the provided case management services to women who use substances during pregnancy during the post-partum period. The program referred women to drug and/or alcohol rehabilitation, and provided specialized instruction for providing care to substance affected newborns. DHMH matched the CHRC grant award and CareFirst awarded a one-time $20,000 grant to the Allegany County Health Department to implement this program. The grantee reported to have served 363 patients and provided 1,592 visits over the duration of the grant.

    Grant Project Description: This one-year grant supported increasing the existing dental program's capacity to serve children with Medical Assistance and to expand access to preventative health services and oral health education for children and their families. Over the duration of the grant, 248 patients were served through 1400 visits.

    Grant Project Description: This one-year grant enabled the Allegany County Health Department to purchase and implement a system which helped to improve the efficiency of the department's patient records and administration while maintaining compliance to HIPAA standards. CHRC grant funding was used to purchase the system and train employees on the use of the system.

    Grant Project Description: This three-year grant supported a collaborative program between the grantee, Allegany Health Right, and Western Maryland Health System to integrate community based mental health and substance abuse services with somatic services for uninsured adults in Allegany County. The program provided patients with referrals to psychiatric services, crisis beds, and outpatient addiction services. Grant funds were utilized to support clinic staff salaries. This program provided services to 470 individuals with mental health or addictions services over the duration of the grant.
