Health Equity Resource Communities/Pathways to Health Equity
UPCOMING DATES OF INTEREST
The deadline to submit applications for the Health Equity Resource Community (HERC) Pathways to Health Equity Call for Proposals was December 13. The Pathways Call for Proposals generated 40 proposals requesting more than $42 million. Applications were received from urban, suburban, and rural communities throughout the state. 19 proposals would serve areas previously designated as Health Enterprise Zones. Over the next few weeks the proposals will be evaluated by a review committee comprised of CHRC Commissioners and HERC Advisory Committee members. Each proposal will be scored on the review criteria listed in the Call for Proposals. Following this review period, a select number of top scoring applicants will be invited to present at a CHRC Commission meeting in February, and grant awards will be made shortly thereafter.
The CHRC has approximately $13 million to award for two-year Pathways grants.

"The Maryland Health Equity Resource Act,” approved during the 2021 legislative session, provides significant new grant funding and state resources for local communities to address health disparities, improve health outcomes, expand access to primary care and prevention services, and help reduce health care costs. The Maryland Community Health Resources Commission (CHRC) will implement this Act with the assistance of the Health Equity Resource Community (HERC) Advisory Committee, the Maryland Department of Health’s Office of Minority Health and Health Disparities, and Chesapeake Regional Information System for our Patients (CRISP).

FY 2021 Call for Proposals - Grants Awarded by the Community Health Resources Commission (Application Period Closed)

Update on Local Health Improvement Coalition Awards
In October 2020, the CHRC awarded 20 grants totaling $1 million to support one-year planning and capacity building efforts of Local Health Improvement Coalitions (LHIC). The grants were designed to support the state's implementation of the Maryland Diabetes Action Plan.
All 20 LHICs submitted Interim Reports to the CHRC in April 2021 and interim/final LHIC reports will be due to the CHRC on October 29, 2021. For any questions regarding the CHRC funding of the LHICs, contact Michael Fay at michael.fay@maryland.gov.

The Maryland Community Health Resources Commission (CHRC) was created by the Maryland General Assembly in 2005 to expand access to health care services in underserved communities in Maryland. The CHRC is an independent commission operating within the Maryland Department of Health (MDH), whose 11 members are appointed by the Governor. Since its inception, the CHRC has awarded 639 grants totaling $98.4 million, supporting programs in every jurisdiction of the state. These programs have collectively served more than 517,000 Marylanders, and grants awarded by the CHRC have enabled grantees to leverage $31.8 million in additional federal and private/non-profit resources.
The CHRC supports the work of community health care resources and fulfills its statutory mission in the following activities:
- Awarding grants to expand access in underserved areas and support public health priorities;
- Staffing the Maryland Council on Advancement of School-Based Health Centers;
- Supporting the State's Diabetes Action Plan through the provision of assistance to Local Health Improvement Coalitions;
- Implementing the provisions of Senate Bill 496 - Recovery for the Economy, Livelihoods, Industries, Entrepreneurs and Families (RELIEF) Act by providing emergency grant funding to Title 7 Developmental Disabilities Administration Providers for re-opening, transformation or revenue loss;
- Establishing the Pathways to Health Equity Program/Health Equity Resource Communities under the Maryland Health Equity Resource Act of 2021; and
- Providing ongoing technical assistance to CHRC grantees and other programs and executing other Special Projects.
In fulfilling these responsibilities, the Commission implements a thorough system of performance measurement to ensure public resources are utilized effectively and efficiently so that Maryland’s underserved communities receive high-quality health care services.
