Addressing Disparities in Social Determinants of Health and Obesity in Maryland Communities

A.  The Maryland Department of Health (MDH or the Department), Office of Minority Health and Health Disparities (MHHD) is issuing this Request for Applications (RFA) to provide community-based interventions to address social determinants of health and obesity in Maryland to reduce health inequity.

B.  The State intends to obtain services, as specified in this RFA, from an Agreement between the selected Applicant(s) and the State. The anticipated duration of services to be provided under this Agreement is one (1) year and one (1) option year.

C.  The Department intends to make up to 50 awards as a result of this RFA.

D.  Applicants, either directly or through their subcontractor(s), must provide all services and meet all of the requirements requested in this solicitation. The successful Applicant shall remain responsible for performance regardless of subcontractor participation in the work. Multiple awards may be given for each Social Determinant of Health (SDOH) and obesity to maximize impact throughout Maryland's jurisdictions.
