We have received several fake computer generated prescriptions in her name mostly for Oxycodone 15mg and Oxycodone 30mg. The police have been contacted however we wanted you to be aware of this as well, We were told to report this to the Board of Pharmacy and that you would put out a letter to all of the pharmacies. We know for a fact that this is happening in Anne Arundel County and Baltimore City but are unaware of any where else it may have taken place at. If you need any other information please feel free to contact us. Jennifer Sacks is the office manager

Dr. Chaudhry's script pad was stolen yesterday out of the office.  She is unaware of who confiscated the pad at this time.  She believes that it could have been stolen from her personal office or in one of the exam rooms.  This incident took place between the hours of 2 and 3pm.  The pack had about 1/2 to 1/3 of the blanks scripts available.  We have notified all of the local pharmacies already and we are hoping that this can be resolved soon.  Thank you for your time in this manner.
