The pharmacist at Kaiser was suspicious, tried to call the clinic to verify. Per Nusheen, the pharmacist called what seems to be a Byaview number and also called again through the switchboard. He had confirmation that the prescription was valid each time. The pharmacist was still suspicious and requested to speak to Dr. Earley. Someone impersonating Dr. Earley confirmed that the prescription was correct and valid. The pharmacist was still suspicious and did not fill the prescription. All this occurred around 1300 8/5/19.
We are still waiting for copies of the forged prescription at JHBMC to follow up. Kaiser has reported the fraudulent prescriptions on their end as well. They will flag all Earley prescriptions and verify with a direct number we provided to them. The clinic confirmed not receiving any phone calls yesterday to verify prescriptions. We are also flagging the associated patient's chart in our electronic medical records.
It seems that a prescription pad has been stolen, but not noticed by the prescriber. The inpatient pharmacy at JHBMC has reached out to the clinic/provider to remove any remaining physical pads as part of the internal investigation. The provider currently primarily uses the electronic medical record to send his prescription orders. This is done primarily electronically, or at times, printed out from the system directly if needed. The fraudulent prescription was written in an "old" prescription pad that is no longer being used.
Can a communication be sent out alerting all Maryland pharmacies about the stolen pads / fraudulent prescriptions that are using Dr. Earley's name? If that was possible, I would also recommend including a direct number where prescriptions by Dr. Earley could be confirmed/verified (410-550-7925). |

The prescriptions are allegedly from Johns Hopkins Medicine and have been written for 5, 10 and 15 mg Oxycodone. The doctors being targeted are Steven John Lahti, MD and Ashot Sergeevich Kotcharian, MD. Several different patient names have been used on the prescriptions. The following names have been identified at this time: Kevin Snead, Cekiara Williams, Anthony K. Wallace, and Terry McLain. It appears that the phone numbers printed on the recovered prescriptions are not the phone numbers associated with the doctors. If you need to verify a prescription that you suspect is fraudulent, conduct a Google search or check your contact lists to identify the proper phone number associated with the doctor. If you do receive a prescription that you suspect is fraudulent, please attempt to contact the diversion investigator assigned to your pharmacy immediately. If you cannot reach the investigator assigned to your area, please call me. At a minimum, please make a copy of the prescription and attempt to obtain identification from the individual that passed the prescription and/or the individual that is attempting to pick it up. If the information involved in this investigations sounds familiar and you have received one of these prescriptions and have not reported it, please report it to your diversion investigator immediately. When the provider (Sarah Jenkins) advised the CVS pharmacy that she had not written any prescriptions for the requested medication/refill, the CVS pharmacist advised that they had several prescriptions on file for Amy Longo under the provider's (Sarah Jenkins/Sarah Wheeler) name. The CVS faxed the copies of the handwritten prescriptions to the provider (Sarah Jenkins) on 6/29/18 & it was clear that the handwritten prescriptions had been forged/were clearly in Amy Longo's handwriting (the provider was familiar with the handwriting since she had worked closely with her in the clinic). Baltimore County & PG County Police were notified, as well as Charlestown Security (the provider now works at the Charlestown Employee Health & Wellness Center in Catonsville, MD). A report/complaint is in the process of being submitted to the Maryland Board of Nursing. |

A second attempt of person (male voice) fraudulently called CVS on 7th st Frederick MD claiming to be me (Dr. Giffi), providing my NPI number to call in an Rx for Phenergan. CVS pharmacist (John) contacted my hospital to confirm the Rx and CVS was notified that no chart existed for this patient and the caller was fraudulently trying to call in a Phenergan Rx. Info called into CVS: Pt info called in: Madeline Payne, DOB 6/7/61, 301-505-6130, 2156 Jones Dr, Frederick, MD 21701 (all fake).
