Kaiser Permanente pharmacy compliance department called Johns Hopkins Bayview Medical Center because on 8/5/19 they received a suspicious prescription for oxycontin written on Dr. Earley's prescription pad. The pharmacist at Kaiser was suspicious, tried to call the clinic to verify. Per Nusheen, the pharmacist called what seems to be a Byaview number and also called again through the switchboard. He had confirmation that the prescription was valid each time.  The pharmacist was still suspicious and requested to speak to Dr. Earley. Someone impersonating Dr. Earley confirmed that the prescription was correct and valid. The pharmacist was still suspicious and did not fill the prescription. All this occurred around 1300 8/5/19.

We are still waiting for copies of the forged prescription at JHBMC to follow up.  Kaiser has reported the fraudulent prescriptions on their end as well. They will flag all Earley prescriptions and verify with a direct number we provided to them.  The clinic confirmed not receiving any phone calls yesterday to verify prescriptions.  We are also flagging the associated patient's chart in our electronic medical records. 

It seems that a prescription pad has been stolen, but not noticed by the prescriber.  The inpatient pharmacy at JHBMC has reached out to the clinic/provider to remove any remaining physical pads as part of the internal investigation.  The provider currently primarily uses the electronic medical record to send his prescription orders.  This is done primarily electronically, or at times, printed out from the system directly if needed.  The fraudulent prescription was written in an "old" prescription pad that is no longer being used.

Can a communication be sent out alerting all Maryland pharmacies about the stolen pads / fraudulent prescriptions that are using Dr. Earley's name?  If that was possible, I would also recommend including a direct number where prescriptions by Dr. Earley could be confirmed/verified (410-550-7925).

Message left for Board as due to COVID they are out of office.  One Prescription written on 05/04/2020 script for Jeremy Sanders DOB 02-02-1977 this is not a valid script- this individual has never been a patient in my care.  This was brought to our attention during a pharmacy flag for a connected party on today's date 10/21/20

The doctor had her prescription pad in her desk drawer. When she reached for it, it was not there.  In looking for it, she found remnants from the prescription pad.  She reported it to the office manager and the manger has contacted local police to file a stolen report and to obtain a case report number.  Local pharmacies have been contacted as well.

Someone broke into the office, subsequently broke into the locked drawer and stole one prescription pad from the box where it was stored.

Office suite was burglarized and 200 scripts were taken.

On 10/21/2020, I was informed that a prescription pad was stolen and used to fill controlled substances for a specific patient that is not and has not been under my active care.

Rite Aid Pharmacy (250 West Chase Street Baltimore, MD 21202) faxed over a request for a prior authorization for a RX for patient Kendall Davis (DOB: 10/08/1984) for Tramadol 50mg 2 tabs po qd #60 with 1 refill. Upon receipt to our office, we identified this patient was the spouse of a MA that used to work for us but was terminated in August 2018. The patient was never seen by Dr. Solomon and he did not authorize this RX.  We think that the MA must have stolen some RX's from the office upon termination, especially since the RX pad used was from our old office location when she worked for us and not our current location.

No physical prescription pad was actually stolen or used, just credentials. The prescription that was requested was not filled. However on 3/11/2021, One Medical received a call from a woman named Betty at Costco Pharmacy in Frederick, MD (ph #: 301-644-1482) indicating suspicion that a patient was providing a fraudulent prescription to be filled at her pharmacy. She faxed over a copy of the prescriptions which are attributed under the prescribing credentials of Michael Gee.

Over the past 2 weeks I have received numerous calls from pharmacies to verify Promethazine with codeine. Although I write for this medication on occasion, some of the names did not match my records. There is a distinct possibility an Rx pad was discarded into the recycling bin accidentally.

I work in Baltimore and currently working remotely. Walmart  in Pasadena alerted me that a falsified / stolen prescription was brought in with my name-  reported to the DEA and made an online police report.

Rite Aid at E. Eager Street called and wanted to confirm if the prescription they had was real.  Devon Allen DOB 10/13/93, presented a prescription for Oxycodone, the prescription paper was an old version and no longer used.  The prescription was not written &/or signed by Dr. Colgan.  This is a fraudulent prescription.

Patient - John Larry Jones came in for emergency visit - reviewed his status -was given Rx Motrin 800mg 20tabs and Penicillin 500mg 28 tabs.  Patient left and stole an entire prescription pad - presented to CVS on Minnesota Ave. Wash, DC (202) 396-2331 for Percocet 30.  Pharmacist called office to confirm this Rx.  Office Manager inform them this was not what was given.  The patient then gave the pharmacist crumbled up Rx for the Motrin and Penicillin.  The office then called back to request the Rx not be filled.  I called Maryland DHMH to report this incident and spoke with Ms. Lisa Guy - Division Chief of Reinforcement.

Upon completion of a credentialing packet for a new job, I was notified that my packet which included a copy of my DEA license was lost or missing. The copy of the license has the information on it that includes my name and  DEA number.  I have also completed a police report for the missing information and will also notify the DEA this morning once they are open. Prescription pad(s) was/were unknowingly stolen from the Riderwood Employee Health & Wellness Center in Silver Spring, MD sometime between November 2011- August 2014.  The pads were under provider Sarah Jenkins' maiden name - Sarah Wheeler - which she hasn't used since August 2014.   Prescription refill requests were received by the provider (Sarah Jenkins) for patient Amy Longo from a CVS in Bowie MD on 6/26/18 & 6/28/18.  Ms. Longo was formerly employed at the Riderwood Employee Health & Wellness Center as a medical assistant.  When the provider (Sarah Jenkins) advised the CVS pharmacy that she had not written any prescriptions for the requested medication/refill, the CVS pharmacist advised that they had several prescriptions on file for Amy Longo under the provider's (Sarah Jenkins/Sarah Wheeler) name.  The CVS faxed the copies of the handwritten prescriptions to the provider (Sarah Jenkins) on 6/29/18 & it was clear that the  handwritten prescriptions had been forged/were clearly in Amy Longo's handwriting (the provider was familiar with the handwriting since she had worked closely with her in the clinic).   Baltimore County & PG County Police were notified, as well as Charlestown Security (the provider now works at the Charlestown Employee Health & Wellness Center in Catonsville, MD). A report/complaint is in the process of being submitted to the  Maryland Board of Nursing.

Mr Gregory Fine, DOB 04/14/1981 stole and has written many RX for himself on medical assistance.  Dr Welsh reported this a long time ago but it is still being filled by pharmacies in Montgomery County.  The phone number on the RX pad is incorrect.

Prescription fraud- Walgreens #12790 contacted us that John Shipley DOB: 3/14/86 presented fraudulent prescriptions for Xanex, Oxycodone and Soma.

The prescription was forged and not signed off by Dr. Goldmark. The police have been called and a copy of the prescription has been provided.

My assistant was called by a Walmart in DC that a Mr Hazzard McCrae presented a fabricated Rx for 30 oxycodone. It is a printed Rx on security paper of a shape and form that our office does not use, includes my DEA and NPI number, office location which can't be found on the internet only internally here at UMMS and signed with nothing like my signature. Thus it is not a stolen Rx pad, but one made from scratch by 'McCrae.' The perpetrator left when the pharmacy called my office to verify.

The Emergency Department received a call from an outpatient pharmacy attempting to verify legitimacy of prescription written on a UMBWMC prescription with an ED physician as prescriber.  It was a fraudulent prescription.  One or more prescription blanks are presumed to have been stolen from the ED.  Police were contacted by the outpatient pharmacy and individual arrested.
