Diagnosis and Treatment (BCCDT) Program

The Maryland Department of Health (MDH) has recently experienced a network security incident. Out of an abundance of caution, most MDH servers are currently offline. There is no evidence at this time that any data has been compromised. However, the BCCDT Program is not able to access or utilize any of our information technology systems. As a result, the BCCDT Program cannot provide information on participant or provider eligibility, status of applications, claim status, or payment status. In addition, The Program is unable to process and pay medical and pharmacy claims due to the server being offline. The BCCDT Program is currently in the process of developing workarounds in order to process new and renewal patient applications and provide application status. We will update you as soon as we have additional information.

For Patients, Navigators, and Coordinators: We are temporarily accepting applications along with all supporting documents via fax (410) 333-7279 and secure email at DLbccdtprovider_DHMH@maryland.gov. Application status requests should also be sent via fax or secure email.

For Providers: We are accepting provider applications and provider revalidations along with all supporting documents via fax (410) 333-7279 and email at DLbccdtprovider_DHMH@maryland.gov. Claim status can be accessed through our provider portal mdeclaims.health.maryland.gov. Claim status requests can also be sent via fax or secure email.


The​ Breast and Cervical Cancer Diagnosis and Treatment Program was established in 1992 to provide breast cancer and cervical cancer diagnosis and treatment services to program eligible Maryland residents (COMAR 10.14.02).  The BCCDT Program reimburses the following providers throughout the state to provide diagnosis and treatment services to program participants: hospitals, physicians, physician assistants, pharmacies, medical laboratories, medical item suppliers, nurse practitioners, nurse anesthetists, nurse midwives, physical and occupational therapists, and home health agencies.

Eligibility requirements for participation in the BCCDT Program

To be eligible for the program, a person must:

  • Be a Maryland resident (proof of residency required);
  • Have a breast or cervical problem requiring further diagnosis and treatment;
  • Be uninsured or underinsured:
    • Uninsured, or
    • Have Medicare, or
    • Have health insurance other than Medicare in which the deductible has not been met, there is a copay or coinsurance, or the needed services are not covered by insurance;
  • Meet the financial criteria established by the BCCDT Program:
    • Income must not exceed 250% of the federal poverty level (approximately $32,200 for a family of one).

Covered Services

The following services are covered under the BCCDT Program:

  • Diagnostic mammograms,
  • Surgical consultations,
  • Breast ultrasound,
  • Breast biopsies,
  • Colposcopies,
  • Cervical biopsies,
  • Surgery,
  • Chemotherapy and radiation therapy,
  • Home health,
  • Pharmacy,
  • Medical equipment and supplies (including prosthesis and bras),
  • Physical therapy,
  • Occupational therapy,
  • Wigs, and
  • Breast reconstruction.

Patient Enrollment Process

Maryland residents can obtain applications to enroll in the Program from the Breast and Cervical Cancer Program Coordinator at their local health department or from the Maryland Department of Health, Center for Cancer Prevention and Control (applications can be downloaded below).  Applications can also be completed at the offices of participating providers.  Proof of Maryland residency and income is required.  Participants in the Program receive an ID letter which should be presented every time they visit their participating healthcare provider.  All bills for services should be sent by the provider to the Department of Health.

Length of Enrollment

Applications for the BCCDT Program must be renewed annually. Please see above notice regarding extension of eligibility related to the COVID-19 pandemic. 

Provider Enrollment

Physicians, pharmacies, home health agencies, outpatient clinics, hospitals, and medical laboratories may enroll as providers in the BCCDT Program by completing the Provider Application and Agreement.

All providers must be licensed in Maryland or in bordering states.  All reimbursement is paid at the Medical Assistance rate or, in the case of Maryland hospitals, at 94 percent of their HSCRC regulated rate.
