The High Intensity Medical Program is a dedicated inpatient medical rehabilitation program providing continuous and intensive medical, nursing, and rehabilitative care to medical complex patients whose severity of illness or injury requires a chronic level of care. Patients require frequent physician interventions, nursing assessments, and ongoing coordination rehabilitation services to achieve their maximum level of independence and discharge to home or the least restrictive environment if home is not possible.
This high level of care includes patients requiring life support such as ventilator management and weaning, and/or renal dialysis. The program also served individuals with activity limitations and participation restrictions due to acquired brain injury.Services are provided through a comprehensive, interdisciplinary team approach, in which all person’s programs are tailored to meet their needs based on their individuality, including culture, age, gender, sexual orientation, spiritual beliefs, socioeconomic status, language, and personal preferences. Our program holds the seal of approval from the Commission on Accreditation of Rehabilitation Facilities (CARF), the top judge of many types of health and human services. This means CARF recognizes our team for their skills and commitment to your safety.
