Maryland Medicaid Long Term Services and Supports enables children to remain with their families and participate in community activities.
Services range from intensive individual support services to nursing services, addressing diagnoses and helping children and families manage challenges.
Through Maryland’s Long Term Services and Supports programs, older adults and individuals with disabilities are able to access personcentered services and supports in their communities that are designed to improve their quality of life.

The Department will submit a 1915(b)(4) waiver application for its 1915(c) Home and Community-Based Options Waiver to the Centers for Medicare and Medicaid Services (CMS) to renew the waiver for five additional years. The 1915(b)(4) relates to selective contracting and allows the Department to limit the providers of case management for the Home and Community-Based Options Waiver to local Area Agencies on Aging (AAA) and agencies selected through a competitive solicitation. HB1696 Draft Rate Study
Section 2 of the 2018 House BIll 1696 requires the Department to compare the Medicaid rate of reimbursement with the actual cost to providers using publicly available information. The Hilltop Institute completed this
draft rate study, which is now open for review.
Maryland Department of Health (MDH) Programs
Hospice ServicesThe provision of hospice care is generally in the home to avoid an institutional setting and to improve the individual’s quality of life during the end of a participants' terminal illness. However, individuals eligible for Medicaid may choose to reside in a nursing facility and receive hospice care in that setting.
The Community Personal Assistance Services Program (CPAS; formerly MAPC), Community First Choice (CFC) and Community Options Waiver all offer a similar service package. Services vary based on eligibility. The core services are personal assistance, supports planning, and nurse monitoring. The Community Options Waiver also offers services in an assisted living.
Medical Day Care Waiver
Medical Day Care is a structured group program that provides health, social, and related support services to functionally disabled adults, age 16 and older.
Model Waiver for Disabled Children
This waiver targets medically fragile individuals including technology dependent individuals who, before the age of 22, would otherwise be hospitalized and are certified as needing hospital or nursing home level of care.
Maryland Health HomesThe program targets populations with behavioral health needs who are at high risk for additional chronic conditions, including those with serious persistent mental illness, serious emotional disturbance, and opioid substance use disorders.

The Family Supports program will provide individual and family supports for children birth to 21 with developmental disabilities on the DDA Waiting List.
The Community Supports program will provide individual and family supports for people with developmental disabilities on the DDA Waiting List.
Community Pathways
The Community Pathways waiver provides services and supports to individuals, of any age, living in the community, meet the criteria for a developmental disability.
