    Calls For Proposals​

    Pathways to Health Equity Call for Proposals

    The deadline to submit applications for the Health Equity Resource Community (HERC) Pathways to Health Equity Call for Proposals was December 13.  The Pathways Call for Proposals generated 40 proposals requesting more than $42 million.  Applications were received from urban, suburban, and rural communities throughout the state.  19 proposals would serve areas previously designated as Health Enterprise Zones. A listing of the applications can be found HERE.​  

     

    Over the next few weeks the proposals will be evaluated by a review committee comprised of CHRC Commissioners and HERC Advisory Committee members.  Each proposal will be scored on the review criteria listed in the Call for Proposals.  Following this review period, a select number of top scoring applicants will be invited to present at a CHRC Commission meeting in February, and grant awards will be made shortly thereafter.

     

    The CHRC has approximately $13 to award for two-year Pathways grants.  ​

    ​​2021 Emergency Relief Act Call for Proposals (APPLICATION PERIOD IS CLOSED)​

    ​​The Maryland Community Health Resources Commission (CHRC) is issuing the Emergency Relief Call for Proposals for Title 7 Developmental Disabilities Administration Providers. This funding is provided to Title 7 providers on a non-competitive, one-time only basis, in accordance with the language of the Recovery for the Economy, Livelihoods, Industries, Entrepreneurs and Families (RELIEF) Act introduced by the Governor and approved by the Maryland General Assembly.  The CHRC will award the funding using a tiered approach based on the applicants’ FY 2019 revenue.  Providers that do not have revenue for FY 2019 but do have revenue for FY 2020 are also eligible to apply for funding.   These funds are intended to help preserve essential service delivery and support re-opening, transformation, or revenue loss (see pages 6 – 9 of the RFP for descriptions or examples of permissible uses of funding).

    The application process is streamlined. Applications will be due to the CHRC no later than 12:00noon, May 10, 2021 via email (mdh.chrc@maryland.gov).   Signed originals are due to the Commission within 30 days of electronic submission of the application.     ​Promoting Community Health Resources: Supporting innovative, replicable, and sustainable projects that serve vulnerable populations and promote health equity​ is the grants program of the CHRC. The program will award grants to community health resources serving Maryland residents. The RFP has the following two strategic priorities:​ ​
    ​(1) Preserving health equity by addressing health disparities and Social Determinants of Health (SDOH), with a particular emphasis on addressing disparities that disproportionately impact racial and ethnic minorities and have been exacerbated by the COVID-19 Pandemic; and
    (2) Promoting the efficient and strategic delivery of integrated population health interventions for vulnerable residents through the support of innovative, sustainable community partnerships that focus on underserved communities, such that the totality of needs for the targeted populations are addressed.

    In this year’s Call for Proposals, the Commission will consider projects in three areas of focus: 

    • Addressing chronic disease prevention and disease management, with specific focus on the prevention and management of diabetes, and the complications and comorbidities of diabetes; 
    • Addressing the health and social needs of vulnerable populations who are disproportionately impacted by the COVID-19 pandemic; 
    • Addressing the immediate and longer-term recovery needs of Maryland's safety net providers as they navigate the impact of the COVID-19 pandemic through efforts that restore capacity to deliver essential health servicesand help support the basic needs of vulnerable communities they serve.

    CHRC Seeking Independent Reviewers for the FY 2021 Call for Proposals

    The Maryland Community Health Resources Commission (CHRC) is seeking to hire independent reviewers in the following areas to evaluate grant proposals as a Maryland Small Procurement.
    (1)  Addressing chronic disease/diabetes prevention and disease management- Small Procurement Category II & III;
    (2)  Addressing the health and social needs of vulnerable populations who are disproportionately impacted by the COVID-19 pandemic - Small Procurement Category II & III;

    ​FY 2020 COVID-19 Virus Emergency Relief Ca​ll for Proposals​ (APPLICATION PERIOD IS CLOSED)​​

    The Community Health Resources Commission (CHRC) has closed the COVID-19 Virus Emergency Relief Call for Proposals. This Call for Proposals aims to assist eligible community health resources respond to the current pandemic.     The Maryland Community Health Resources Commission has released its FY 2020 Local Health Improvement Coalition Request for Proposal. This request for proposal is only for Local Health Improvement Coalitions.

    LHDs/LHICs must submit the application forms to the CHRC no later than Wednesday, October 14, 2020, to lhic.fy2020rfp@maryland.gov.

    The CHRC Commissioners will issue grant awards at their meeting in late October 2020.

The program awards grants to community health resources serving Marylanders in Maryland. In this offering, the Commission considered projects in one priority area. The Commission’s priority area for Fiscal Year 2010 was reducing infant mortality, pre-term births and low birth weight babies through pre-natal initiatives.

    The Maryland Community Health Resources Commission released a 'Call for Proposals' for Operational Grants on Friday, October 19, 2007. The priority areas included:

    1. Services to individuals with co-occurring mental illness and substance abuse
    2. Diversion of non-urgent care from hospital Emergency Departments
    3. Services to immigrants with an emphasis on establishing a 'medical home' for these individuals.

    In this offering, the Commission encouraged projects that will improve access to dental services for low-income, uninsured individuals and Medicaid recipients in new and creative ways. The focus is on, but not limited to services for families and low-income children. The Commission awarded grants focused on creating new or expanding current sources of dental care to community health resources in Maryland.
